In order to receive an entry, you must be a member to participate. Please complete the survey below accurately and completely including the employees name or teller #, your name, full account number, and the date & time of your interaction. Each valid survey you complete will earn 1 entry into the $250 quarterly drawing (you are allowed up to 3 entries per quarter, per member). $250 Member Survey Quarterly Drawing Official Rules and Requirements
Membership required to participate in this promotion. $250 Member Survey drawing will be held on the last day of each ending quarter unless said day falls on a non-business day. If drawing day falls on a non-business day, drawing will be held the next business day. All surveys must be submitted by 5:00 PM CST on the last day of the current quarter in order to be accepted for that quarter’s drawing. (dates listed below)
To receive an entry; the entrant must be a legal resident of the 50 United States and District of Columbia, and is 18 years of age or older. The entrant is NOT an employee, director, or volunteer at Members First Credit Union. The entrant is NOT an immediate family member and/or in the same household of a Members First employee, director, or volunteer. The entrant must be a current member in good standing with Members First Credit Union that has performed a transaction or service with an employee at any Members First Credit Union location. The entrant must complete the survey within seven (7) business days of the transaction receipt unless it is the last day of the quarter, then the entry must be submitted by 5:00 PM CST on the last day of the current quarter in order to be accepted for that quarters drawing. If entry is submitted after seven (7) days of the date of the transaction or interaction or after the quarter has ended, the survey will be void and no entry will be recorded. If there is no employee listed on the survey or any portion of the survey is inaccurate, untrue, left blank, filled with N/A (unless otherwise directed), don’t know or don’t remember, the survey will become void and no entry will be submitted into the drawing.
After the survey has been verified for accuracy & met the guidelines, the member will receive (1) one entry for each completed and verified survey, up to a maximum of (3) three entries per quarter. The entry will be submitted for the current quarter that the survey was completed. On the last day of each quarter (Dates listed below), (1) One winner will be selected in a random drawing to win a $250.00 prize. Odds of winning depend on the total number of surveys submitted each quarter.
The winner will be notified by phone within 10 business days of the drawing each quarter. In the event we are unable to contact the winner for any reason or the winner has not responded to Members First Credit Union within 24 hours of contact, we will randomly draw an alternative winner. When the winner is selected, the $250.00 prize will be credited to the members account number associated with the survey entry (no cash or gift cards will be given). By accepting this prize, the winning member may be asked for proof of transaction by collecting their transaction receipt. The winning member will agree to sign a consent disclosure for Members First Credit Union to use the winning members name for all promotional materials related to the $250.00 survey drawing if required. All decisions are final and at Members First Credit Union’s discretion.
By accepting the prize, the winner will not be able to win the Survey drawing again within 365 days from the win date. The winner of this prize agrees to hold Members First Credit Union and its directors, officers, and employees, harmless against any and all claims and liabilities arising that may be caused or claim to be caused out of the acceptance or use of this prize. Members First Credit Union is not responsible for any incorrect or inaccurate information. Members First Credit Union reserves the right at its sole discretion to cancel, terminate, modify or suspend this promotion at any time.
Quarterly dates are as follows:
1st Quarter – January 1st to March 31st
2nd Quarter – April 1st to June 30th
3rd Quarter – July 1st to September 30th
4th Quarter – October 1st to December 31st
